Please fill out this form, whether or not you are a registered patient with Amber Dental or Gisborne A&E Dental. Please contact the practice after you fill out this form to let us know you have done so. You can do this by emailing us at office@amberdental.co.nz. Please DO NOT come to the practice without being asked to do so. We are NOT seeing ANY walk in patients. If you are asked to come to the practice, please arrive only at the advised time and remain in your car in the parking lot. Please CALL and advise the practice that you have ARRIVED in the car park. We will CALL you back to ask you to come to the door when we are ready to receive you, please keep your cellphone handy. No attenders or family members will be allowed into the practice unless the patient is a child or is elderly. Please do not bring any personal items to your appointment like jackets, purses, bags, hats, caps, sunglasses etc. Please carry only a wallet at most with your personal items like your phone, keys etc. If you are found carrying a large personal item that you cannot store in your pocket, you may be asked to return to your car and leave it there. When you come into the practice, please DO NOT touch anything including inside door handles etc. You will be guided to the room where your treatment will be provided after hand sanitisation and provision of a mask. Once your treatment is completed, you will be required to pay for your treatment through EFTPOS. If you are under 18 years of age or if your payment is being provided by WINZ or ACC, you may or may not be required to pay anything. You will be guided out of the practice after completion of your treatment, please do not leave of your own accord or touch anything when you leave. Thank you for your understanding.
If you are not a patient who is registered with us, please go to the Documents link at the bottom of any page on our website and register with us by filling out a registration form and a medical history form. This can be done AFTER you fill out this form. We will not be able to see you or give you a prescription until you have completed our registration process.
